If you have collected money on behalf of the PTA, please complete this Deposit Voucher.  Please be sure all checks have been made payable to "FKPTA" and are signed prior to submitting your deposit.

 

If you have purchased something on behalf of the PTA and require reimbursement, or if your committee requires a payment be made directly to a vendor, please submit this Expense Voucher.

 

If you are hosting an event and require start-up cash, please fill out this Cash Box Request Form.  This form can also be used to repay a cash box loan.
